Hospice is specialized care designed to support individuals and their loved ones during a life-limiting illness. The focus of care is on comfort and quality of life. Hospice care addresses the medical, physical, emotional, spiritual, and social needs of each patient while also being there to support their loved ones. The goal is to enhance our patient’s quality of life while vigilantly managing their overall health.

To be eligible for hospice care, our Hospice Medical Director or the patient’s doctor will certify that the patient's health has declined enough to qualify. Usually this results from a terminal illness or advanced stage illness and the related symptoms mentioned in the diagram to the right. If those symptoms look familiar, you could be missing out on resources and medical care at no cost to you. It's time to ask us to come to you for a simple assessment. A registered nurse will perform an evaluation to assess the patient for eligibility criteria. Hospice-appropriate individuals may exhibit some or all of the following:
Frequent hospitalizations or ER visits
Progressive weight loss
Deteriorating mental capacity
Recurrent infections
Overall decline in condition
